Rehabilitation Therapy

Our experienced therapy team takes a resident-centered approach to interventions to target the mobility and functions needed to progress toward discharge and/or a higher level of function.

In-House Therapy

Our Vision — Your Success

Marianwood houses two fully stocked therapy gyms as well as mobility bars and training stairs. We provide necessary mobility equipment during your stay (walkers, wheelchairs, etc.) as needed and will assist you in ordering your own equipment through your insurance provider when applicable. We utilize a variety of methods including one-on-one and group therapy sessions to help residents achieve their goals.

The fully in-house therapy team includes Physical, Occupational, and Speech therapy services.

At Marianwood Health & Rehabilitation, our therapists take pride in their practice and expertise. Our in-house therapy program works collaboratively with other team members including nurses and physicians, to create a treatment plan that meets each person’s unique needs and goals.

Our Holistic Approach

Marianwood Health & Rehabilitation’s unique collaborative environment supports patient centered outcomes across all areas of care. Our therapists, providers, and nurses share recommendations and train other disciplines to ensure the continuity of care throughout a resident’s stay.

Rehab to Return Home to your Community

Anticipating Your Every Need

We work with each patient, their family, caregivers, and their healthcare providers to determine the best care and treatment plan. When the goal is to return home, we explore every detail about our patient’s prior living arrangements. Family members/Caregivers are encouraged to participate in care and therapy sessions so that they can help their loved one prepare for discharge.

When the goal is to discharge to a supported setting (e.g. assisted living, adult family home, etc.) we collaborate with community partners to coordinate a safe and supportive discharge.

Our Long-term Care residents receive quarterly check-ins with the therapy department to address any changes, concerns, or new goals that may arrive during their time living here.

Your Rehab Team

Many Disciplines — One Goal

The therapy team consists of physical therapists (PT), physical therapy assistants (PTA), occupational therapists (OT), certified occupational therapy assistants (COTA), speech and language pathologists (SLP), and a rehab technician, who work together based on the individual needs of each patient.

The Physical Therapist assesses and treats impairments in strength and function that occur as a result of injury, disease, age-related degeneration, or environmental factors. Physical therapy treatments are performed by the physical therapist, or a physical therapy assistant who treats under the direction of the supervising physical therapist.

The Occupational Therapist assesses the impact that a decline in function has on a person’s’ ability to performe everyday tasks such as dressing, bathing and using the bathroom as independently as possible. Occupational therapy treatments are performed by the OT (or a COTA working under the direction of the supervising OT).

The Speech Language Pathologists assesses language, cognition, and swallowing dysfunction. Speech Therapist’s work on a wide variety of skills like safe swallowing, memory, problem solving, speech, and language.

These three disciplines work together along with nursing, social services, activities, the business office, families, caregivers, and local community partners to assess functional potential, and collaborate on the unique goals for every patient. Family, friends, and carevigersare essential part of the interdisciplinary team, as they provide the background and detail of their loved one’s life to incorporate into our plan of care.

On-Site Decision Makers

We Don’t Wait To Make Important Decisions

In order to provide the best care possible, our caregivers must be empowered to make decisions, because in many cases, every second counts. So rather than waiting for a decision from higher-ups who might not understand the crucial details of any given situation, we leave the decisions to those who are closest and know best: the caregivers, doctors, and nurses who work with our patients 24-hours a day, 7-days a week.

It’s important to us that our patients, as well as their family members, feel loved, informed and comfortable during their stay at our facility. Quality of life is just as important as quality of care in a skilled nursing facility – and we work hard to make sure that every patient is as happy and comfortable as possible.
